Dr. Memar in downtown Chicago uses a revolutionary new breast augmentation technique that results in very natural breast shapes with less scarring and less chance of complications vs. using standard breast implants. Breast implants filled with saline or silicone have been the norm for breast enlargement or augmentation. In the 1980s liposuction was created for the surgical removal of unwanted, localized body fat. These fat cells were moved to other parts of the body, through a procedure known as Autologous Fat Transfer, to replenish depleted or depressed acne scars, aging or underdeveloped lips, or simply to enhance body contours. Over the years, injecting a patient’s fat into their breasts has become the most “Natural Breast Augmentation” performed in our office.

The Issues With Implants

Historically, breast augmentation procedure required the use of saline or silicone implants. While many people have received this procedure and have been thrilled with the results, there are complications and risks one should be aware of before going ahead with silicone butt implants. Implants can become dislocated , there is a risk of infection when implants are used (versus fat relocation), There can be a capsule that is created around the implant and this capsule can shrink and cause cosmetic and medical problems. The capsule can also hide potential breast cancer making early detection a problem. Furthermore, once you commit to an artificial breast implant, you need to keep redoing it every number of years and as your breast tissue ages and the skin thins, there is a posibility that the implant can become visible through the skin and give an unnatural appearance. This is seen in older women. If the implant is removed at this time, it will give a pancake like breast with great deformity.

The Safer Option: Fat Transfer

Conversely, fat relocation procedures, during which the surgeon uses the patient’s own fat to enlarge and shape the breast, has far fewer potential complications than implants. Because the patient’s own fat is used, an autoimmune problem is not a concern. Becoming visible through the skin is not a possibility.

Originally, researchers believed that fat cells would not survive long term after implantation to the breast. But more recent studies have shown that fat transplants do survive: depending on the patient’s health, about one-third of fat transplanted into the breasts will survive long term. Secondly, some fat cells can die and leave calcification. Original mammography could not tell this form of calcification from malignant calcification. That lead to unnecessary biopsies. However, the newer technologies in mammography can distinguish the two. It is imperative not to smoke, because smoking kills the fat grafts.

Your Consultation for a Brazilian Butt Lift

As with all of our cosmetic procedures, your first appointment at Dermatologic Cosmetic & Plastic Surgery will be a consultation with your surgeon. You will discuss your medical and health history with your doctor and the doctor will talk to you about your expectations of the surgery. You will meet your surgeon and assistant. Detailed discussions of the procedure will be made. The doctor will order a pre-operative workup by an internist, including blood tests and EKG of the heart. Only if the tests are normal, can a procedure be done. He will discuss the procedure, healing, and when to expect a final result.

The fat Transfer Breast Augmentation Procedure

A fat transfer to the breasts generally takes between two and three hours to perform, however you will be at the center for about 5 hours. The doctor will draw on your body, identifying the areas that will be suctioned, the insertion sites and the areas that will be enlarged. The degree to which you are anesthetized will vary depending on the extent of the procedure and your doctor will discuss this with you ahead of time. Pre-operative medications are administered and the area is anesthetized using Klein’s tumescent solution. After the areas are numbed, the fat is liposuctioned from the unwanted areas. The fat is purified in a sterile setting. Next the fat is pumped in a closed system back into the body. This time, the fat is placed in multiple layers in the breast, including under, in, and over the muscle. It is important to know, some surgeons use syringes to transfer the fat, and our closed system reduced the risk of infection by minimizing human contact with the fat. After the fat has been pumped into the breasts, a compression garment is placed on the patient. This garment needs to be worn for a total of 3 weeks.

Potential Problems With the Procedure

The number one risk is an infection. Therefore, we like to have the patient wash with chlorhexidine for one week before the surgery. we also want mupirocin ointment to be placed in the nostrils for one week daily before the surgery date. There will be small scars at the sites fat has been removed. The scars are the size of a pencil head eraser. Your doctor will show where the scars will be and the sites can be placed in areas that are less noticeable.

Recovery from Surgery of the Breast

As with most cosmetic surgery, you will likely experience some swelling and mild to moderate pain. This pain will be initially relieved with narcotic pain killers and then OTC Tylenol. Your doctor will recommend a pain reliever to you.

I recommend taking one week off of work and slowly resuming your activities. You can go back to the gym in 3-4 days, but do not sit down and listen to your body; do not over exert.

 

You will be swollen for a few weeks, and then the swelling will start to go down. Remember that fat has been placed in the buttocks. This fat does not have a blood supply yet. I mean there is no blood vessel connected to it. So it will survive by consuming the fat it is storing and the fluid it can absorb from its surroundings. Therefore, that fat cell will shrink as it consumes the lipid droplet it is storing. In this process, it is sending out feelers to reconnect to the local blood supply. After it has reconnected, it can now start absorbing blood lipids and enlarging. This process can take many many months. Therefore, your final results can take 12-18 months to fully present.

The fat, which can be taken from your belly, buttock or even from under the arm next to the breast is transferred to your breasts using advanced microsurgical techniques. Since it is your soft, living tissue you are not at risk for many of the problems seen with implants.

How is the procedure performed?

Your fat is harvested using a minimally invasive procedure like SmartLipo or Vaser. First the donor site is numbed with a local anesthetic. Then our doctor uses low-vacuum liposuction to harvest your excess living fat cells. The fat removed is prepared and then re-injected into your breast tissue. These new transfer techniques have shown that up to 70% of the transferred fat cells will survive leaving your breast with the fuller shape you desire…

What other conditions respond well to Natural Breast Augmentation with fat?

Those patients with postpartum loss of fullness or with a mild breast droop and those patients who want to restore their breast shape and/or cleavage are equally satisfied with their results from Natural Breast Augmentation with fat.

Are there limitations to the amount of increase I can have with Natural Breast Augmentation?

Yes. Natural Breast Augmentation with fat transfer is not for everyone. The average breast enlargement that retained good, long-term results was between 1 and 1.5 cups. Patients wishing to go from an A cup to a D cup would be best served by implants.

Will fat injected into my breasts interfere with mammograms?

No. With today’s advanced radiologic screening techniques it is easier for radiologists to distinguish between the changes associated with breast cancer from benign necrosis of breast tissue. Many studies support the idea that radiologists have a high level of confidence in differentiating between fat necrosis calcifications after breast surgery and those related to breast cancers.

Will I have to have more than one procedure?

Most patients are satisfied with their results after only one treatment. However, about a third of our patients request an additional fat transfer procedure to achieve their desired amount of fullness.

Who are the best candidates for Natural Breast Augmentation?

The best candidates include patients that would like breast augmentation but don’t want implants, and those patients who have had implants in the past but are unhappy with the results.
