Brazilian women are known for their beautiful backsides, often proudly displayed in their revealing signature swimsuits. But not all Brazilian women are born with beautiful backsides. Brazilian women have a secret: the Brazilian butt lift (aka, butt augmentation).
Traditional butt lifts remove excess skin and fat from the backside area and are often performed on people who have lost a significant amount of weight. Conversely, a Brazilian butt lift shapes the backside area by injecting or grafting fat from other areas of the body (like the abdomen, hips, or thighs) into the buttocks. A skilled cosmetic surgeon can then not only tighten the skin, but actually enhance the shape of the patient’s buttocks. The Brazilian butt lift is quickly becoming one of Dermatologic Cosmetic & Plastic Surgery’s most popular procedures for both men and women. Some of our patients have lost weight – and lost their butt in the process. Others have found that aging and sitting in an office chair for hours on end has made their behind flat and droopy. Others have simply never liked the shape of their buttocks.
Why is the procedure called a “Brazilian”?
The Brazilian butt lift is actually not named after the stereotypical Brazilian backside. Instead, it is called the “Brazilian” butt lift because the procedure was pioneered and perfected by Brazilian plastic surgeon, Ivo Pitanguy. Dr. Pitanguy, a native of Brazil, trained in the United States, France, and England, before returning to Brazil to practice in the 1950s. Although he may be most well known because of his development of the Brazilian butt lift, Dr. Pitanguy was a philanthropist who treated poor children, burn victims, and other patients for whom plastic surgery was a life-saver. That being said, he was for years the primary plastic surgeon to the stars, including Elizabeth Taylor and Sophia Loren. Dr. Pitanguy passed away in August of 2016, only a few days after carrying the Olympic torch during the opening ceremony of the Games. He leaves behind a legacy of humanistic philosophy and treatments in the field of plastic surgery.

Brazilian Butt Lift (BBL)
Baby got back. The buttocks is very important as a cushion to the pelvis and spine, supports good posture, and serves as an important sexual organ. A number of factors can shrink our toukhas, including age, genetics, weight loss, diabetes, some infections and the medications for some infections like HIV meds, and trauma. So how is one to regain or enlarge one back side?
Solid silicone implants are available to be surgically placed under the muscle or skin of the buttocks. The problem with these include an increased chance of infection, dislocation of the implant, nerve damage, and disfigurement. Secondly, this entails a major procedure that is done under general anesthesia. With weight change, and further thinning of the buttocks skin, the implant can start to look and feel awkward. Im not a big fan of this procedure and I’ve seen one too many cases of severe infection after this implant.
The other approach to enhancing the back side is the use of a filler. A filler is an amorphous mass that is meant to be placed under the skin and inflate the skin. Fillers used can include liquid silicone (Silikon 1000) or ones own fat tissue acquired through liposuction. The other known fillers can also be used, however the amount needed can be quite cost prohibitive. If money is not an object, then this might be the easiest way of buttocks augmentation. Outside of this country, a filler called Bio-Alcamid has been employed in buttocks augmentation. However many longer term studies are showing that the filler can get infected much easier than previously thought and there have been instances of filler migration.
In my opinion, if the patient has enough fat to transplant to the buttocks, this is the ideal option.
Who is a good candidate for a Brazilian Butt Lift?
The ideal patient is in good health. The patient needs to have fat to liposuction. The ideal place is the abdomen, lower and upper back, the arms, posterior and inner thigh. Most patients are women, however men can benefit from this procedure.
What do I have to do before surgery?
Before getting the procedure, a medical clearance is necessary from a primary physician, including a normal EKG. If you smoke, you need to stop smoking for at least 2 weeks before surgery and you cannot resume smoking for at least 6 months after surgery. Hopefully you wont restart smoking. One week before surgery, get rid of all blood thinners, including alcohol, aspirin, Ibuprofen and other NSAIDS, fish oil, high dose vitamin E, and most Chinese herbs. Bring your garment for a fitting at the office a few weeks before the surgery. You need to wash your entire body, including scalp with Chlorhexidine anti-bacterial wash. I also like an antibacterial called Mupirocin ointment to be placed in the nostrils every day for the 7 days before surgery. Bring the medications and garment with you on the day of surgery. You will need someone to pick you up after surgery. You cannot go home by yourself and you will not be able to drive that day after surgery.
The surgical procedure
A BBL is a fat transfer procedure where fat is removed from the unwanted area using liposuction. The fat is placed in the buttocks at different layers, including under, the muscle, in the muscle, and over the muscle. I do not like using syringes for the reintroduction. I used a closed system, and the fat is pumped into the buttocks. The fat is not manipulated. At the same time, fat is removed from around the buttocks to give the buttocks a shape and projection.
A garment is need to be worn for at least 3 week. It is important not to sit on the buttocks for at least one week. In this period, pain killers are prescribed along with antibiotics. The area will continue to change and final results can take over one year to present.

The Road to a Perky Buttocks
The desire to have a “perky backside” is a hot tipic in American culture and around the world. But the truth is that having a high, firm, smooth behind is more genetic than anything. Yes, you can do certain exercises that will help tone and tighten your buttock muscles, but at the end of the day, you might just not be genetically inclined to have that “perfect” butt. Throw in weight loss and the affects of aging, and you might be left with quite the opposite of what you want in a behind-side.
The Issues With Implants
Historically, buttocks augmentation procedure required the use of solid silicone implants. While many people have received this procedure and have been thrilled with the results, there are complications and risks one should be aware of before going ahead with silicone butt implants. Implants can become dislocated (especially in an area that experiences as much movement as the buttocks), there is a higher risk of infection when implants are used (verses fat relocation), and both of these things can lead to potential nerve damage and disfigurement.
The Safer Option: Fat Transfer
Conversely, fat relocation procedures, during which the surgeon uses the patient’s own fat to enlarge and shape the buttocks, has far fewer potential complications than implants. Because the patient’s own fat is used, allergic reaction is not a concern.
Originally, researchers believed that fat cells would not survive long term after implantation to the backside. But more recent studies have shown that fat transplants do survive: depending on the patients health, about one-third of fat transplanted into the buttocks will survive long term. It is imperative not to smoke, because smoking kills the fat grafts.
Your Consultation for a Brazilian Butt Lift
As with all of our cosmetic procedures, your first appointment at Dermatologic Cosmetic & Plastic Surgery will be a consultation with your surgeon. You will discuss your medical and health history with your doctor and the doctor will talk to you about your expectations of the surgery. You will meet your surgeon and assistant. Detailed discussions of the procedure will be made. The doctor will order a pre-operative workup by an internist, including blood tests and EKG of the heart. Only if the tests are normal, can a procedure be done. He will discuss the procedure, healing, and when to expect a final result.
The Brazilian Butt Lift Procedure
A Brazilian butt lift generally takes between two and three hours to perform, however you will be at the center for about 5 hours. The doctor will draw on your body, identifying the areas that will be suctioned, the insertion sites and the areas that will be enlarged. The degree to which you are anesthetized will vary depending on the extent of the procedure and your doctor will discuss this with you ahead of time. Pre-operative medications are administered and the area is anesthetized using Klein’s tumescent solution. After the areas are numbed, the fat is liposuctioned from the unwanted areas. The fat is purified in a sterile setting. Next the fat is pumped in a closed system back into the body. This time, the fat is placed in multiple layers in the buttocks, including under, in, and over the muscle. It is important to know, some surgeons use syringes to transfer the fat, and our closed system reduced the risk of infection by minimizing human contact with the fat. After the fat has been pumped into the buttocks, a compression garment is placed on the patient. This garment needs to be worn for a total of 3 weeks.
Potential Problems With Brazilian Butt Lifts
The number one risk is infection. Therefore, we like to have the patient wash with chlorhexadine for one week before the surgery. we also want mupirocin ointment to be placed in the nostrils for one week daily before the surgery date. There will be small scars at the sites fat has been removed. The scars are the size of a pencil head eraser. Your doctor will show where the scars will be and the sites can be placed in areas that is less noticeable.
Recovery from a Brazilian Butt Lift
It is imperative not to sit down for one week after the surgery. Some patients choose to not sit for an even longer period, but my requirement is a minimum of one week. As with most cosmetic surgery, you will likely experience some swelling and mild to moderate pain. This pain will be initially relieved with narcotic pain killers and then OTC Tylenol. Your doctor will recommend a pain reliever to you.
I recommend taking one week off of work and slowly resuming your activities. You can go back to the gym in 3-4 days, but do not sit down and listen to your body; do not over exert.
You will be swollen for a few weeks, and then the swelling will start to go down. Remember that fat has been placed in the buttocks. This fat does not have a blood supply yet. There is no blood vessel connected to it. So it will survive by consuming the fat it is storing and the fluid it can absorb from its surroundings. Therefore, that fat cell will shrink as it consumes the lipid droplet it is storing. In this process, it is sending out feelers to reconnect to the local blood supply. After it has reconnected, it can now start absorbing blood lipids and enlarging. This process can take many many months. Therefore, your final results can take 12-18 months to fully present.
